By sharing your story, you give Psanctuary Church permission to use your words, voice, image, and name (as you choose in the form below) in its ministry. This includes our website, social media, emails, printed materials, recordings, and events.
We may shorten or lightly edit your story for length or clarity, without changing its meaning. Sharing is voluntary and unpaid.
This permission has no end date. You may ask us to stop using your story at any time by emailing info@psanctuary.org. We will remove it from places we control going forward, but we cannot recall anything already printed or shared.
By checking the release box in the form, you confirm that you are 18 or older, that this story is your own, and that you release Psanctuary Church from claims connected to the use of your testimony as described here.
